Below you will find both forms needed at ALL of your Littlest Heroes Sessions.

PARTICIPANT RELEASE FORM

-Must be signed by the parents at the time of each littlest heroes sessions you conduct. A scanned copy of the document must be sent information@littlestheroesproject.org after your session along with 5 of your favorite images from the session. Please keep all of your release forms and photos together after each session. If asked, we may need the original copy(s) sent to us.

INTERVIEW FORM

–Print and take this document with you for each littlest heroes session. After your session takes place you will be asked to send in your photos as well as present us with the answers that are presented on this interview form. This is a way for you as well as the organization to get to know the child and family before sharing their story on our website. We ask that children over the age of 4 try to answer the questions directed to them so we can truly get to know the hero inside! This is another fun way to make the children feel extra special during their session. Make them feel like a star!

Things to remember:

- Do not forget to email us the child’s release and interview forms after your session ( information@littlestheroesproject.org )

-All we need from you is 5 of your favorite photos from each session (high resolution is best) with NO matermarks

-You need to present the family with at least 10 high resolution photos (free of charge) in print or CD form and a free sitting/session fee. If you wish to provide the family with more photos and or/extras that is up to you, the photographer to decide.
